Birth and Death Records

Birth and death certificates are not maintained at the county level in Oklahoma. The Oklahoma Vital Records Service under the Oklahoma State Department of Health is the custodian of these records. Birth certificate fees include $15.00 for a standard certificate, $35.00 for an heirloom certificate, and $40.00 for a delayed birth certificate. Death certificates cost $15.00 per copy. Birth records are restricted to eligible applicants — generally the individual named on the record, an authorized representative, or a person acting under court order.
